In our area the DSM encourages DOMs and Unit Safety Reps to meet at least once a month to review health and safety matters. The purpose of the monthly meeting is to formalise any issues that can then be actioned by the DOM and Safety Rep working together to change a particular bad practice or introduce better practices.

That doesn't mean the Unit Safety Rep cannot approach the DOM at any other time if there is a H&S issue at the office. With the best will in the world DOMs will not have the same safety knowledge that Safety Reps can access and take interest in.

I am tempted to say when ever you want one!
However Royal Mail say at least 10 a year with 1 a month being the norm.
