It wasn't safe placed, it just said tracked on the label so delivering a tracked to a external postbox is ok. Unfortunately for me it turned out to be an unmarked age verification.

As others have said you still need a photo of the item half in/half out of the box, unless you did that before noticing it was a AV?
How did this get to the managers attention, as it's surprising that a routine customer would go to the trouble of complaining about a parcel left in a postbox even if it was an AV item?
The managers do spot checks as they know how many are in the office and on what delivery. They check photos and dates of birth, a couple in our office have found out the hard way, I think one of them was using his own birthday when safeplacing them.

Exactly, when the person delivered the item they just guessed and put a random date along with faking the signature when they either posted or doorstepped/chucked it by the bin.

Definitely. We were told there had been a statistically unusual spate of AV items delivered with the same age input. Although it was a 1930s DOB, so maybe someone in their late 80s is sipping whisky every day to make it to 100.
The fella made a mistake and is highlighting it for others. Some amount of absolute weapons on here who by the sounds of it have never made an error in their lives.
Seems the penalty is punitive and based on the system being absolute.
Did you or rep and manager check what information is generated on the pda.
In a case I dealt with we did recreate the situation and discovered the pda didn't register it as an age verification item.
So in this case it was item Id issue and so no further action required in relation to the conduct investigation.
It went through the process.
So what did you do? You still havent said.
You scanned the item. Put it in the postbox. Went to take a picture of the item and.......?
He couldn't get it back out!!!! - I'm sure you've seen them boxes before
I havent tbh - the smaller letterboxes that can only fit slim packages I take a photo with the item only halfway in so it would be retrievable. Ive never seen a bigger parcel box I couldnt get the parcel back out of.
Besides that, what im really asking is did he call his manager as soon as he realised he couldnt get it back out or did he put in fake details and a fake signature and go on his merry way

Putting in the fake details and signing for it himself is more than just an accident - you could get away with calling it an accident if you immediately phoned the manager and explained what had happened and asked what you should do next
If you tolerate this, then your paid break will be next

I dropped it straight it into the box thinking it was an ordinary tracked ready for it's picture.
By that point there was nothing I could realistically do.
It's very easy to do with an external postbox, if you don't know it's A.V
Scan barcode
Put parcel in postbox
Go to take picture of item in postbox ( As per tracked guidelines)
See it's A.V on PDA.
Screwed.
Item should only be partially placed (or held) inside the postbox until such time as the photo is taken. As such, the photo prompt is AFTER the AV prompt, so how co it be that an AV is delivered mistakenly without 1st entering the year of birth?

As mentioned, they have a list of AV items and I think someone checks them manually. Number of us in my office had a phone call from our manager saying it was a heads up that it had been noticed some had been inputting the same year of birth for multiple items.
In my case it was 2 items 3 days apart in completely different towns with I think year of birth 1968 or something. I didn't do anything wrong and it wasn't a direct accusation but there was a slight implication that I'd simply entered that date without asking the recipient.
We've also been told that a number of AV items could be from secret shoppers who will obviously notify if an AV item hasn't been delivered as per our operating procedure.
