How did this get to the managers attention, as it's surprising that a routine customer would go to the trouble of complaining about a parcel left in a postbox even if it was an AV item?

This is more of a be careful thread guys. Not a how did it happen.
Don't rush.
Don't take too much.
If you can't complete ask the managers what they want you to do.
And above all be careful and check things twice, if in doubt take it back to the office.
I've learnt my lesson now regrettably.

They get a notification of any av items that have arrived in the office, ours tells us that we have one for whatever address and reminds us to stick to the procedure so maybe the manager checked it had been delivered as it should and discovered that it hadn't.

In a case I dealt with we did recreate the situation and discovered the pda didn't register it as an age verification item.
So in this case it was item Id issue and so no further action required in relation to the conduct investigation.
It went through the process.
You scanned the item. Put it in the postbox. Went to take a picture of the item and.......?
If you tolerate this, then your paid break will be next

Exactly.
It was an honest mistake, I didn't cover it up and I got the punishment for it.
My own fault for trying to do 2 days mail and clear when no overtime allowed.
I've learned to go at average pace and check what I'm doing. If it doesn't get completed it's not my decision what happens to what's left over.

It’s also a clear warning to never attempt double mail during your contracted hours, either extend if allowed, or tell them to do one. Dont rush it.

My former partner who has since retired taught me a good lesson. He said ‘no point being a hero you won’t get any thanks for it.’ Never has that been more true especially with little or no overtime available just do the minimum but do it well.

The managers do spot checks as they know how many are in the office and on what delivery. They check photos and dates of birth, a couple in our office have found out the hard way, I think one of them was using his own birthday when safeplacing them.

On the subject of AV items, more needs to be done to educate the buyers that what they are buying is an AV item. It's getting a bit tedious explaining to people that you need their birth year only for them to look at you as if you've committed a grave offence against them. "you've bought an age restricted item". "no i've not'.

Ive always taken a pic of tracked in the box with the lid up.

Don’t safe place them just take them back if no answer. As for getting a good photo, good luck! The pda is constantly freezing so you get what you can until that’s fixed.
