Hi had part E lifetime allowance form sent back to me to fill in not sure what information to put down on the form can any one help please

I had issues with this too,I phoned them up and they helpfully gave me the percentages and figures to write in as I couldn’t find them all.

Have found one on the forms NA6O 19% and my annual pension cannot find the % on the cash balance fund got the annual pension or year , I’ve phoned them up keep passing the book
