Footwear medical note.
Having had a medical note a few times until last cpl years when last dom made it impossible trying to get the payment through , i asked the medical practice about it. Got an email stating theres a £30 charge for any note regarding this , which obviously wipes out any royal mail payment. ! Strange times .

You should have a fit note explaining what's wrong and what adjustment you require. If you have seen a specialist they will have will have diagnosed what's wrong and put that in writing. That should be sufficient.
