Disagreement form?

Hi All,
Is there a form for going to disagreement?

I have had a look in the download section but have had no luck.

Are you sure that you want a Disagreement Form? Maybe a Grievance form might more appropriate (attached) let me know how you get on :wave :wave

As far as I know there is no specific form, but when I helped my rep all he did was a covering letter to the Managers with a brief description of the problem and the hope that a solution could be reached.

then he drew up a table with 5 columns,

1st Column was a description of each point
2nd Column was what he thought Royal Mails position on that point was
3rd Column was what the Union.Reps/Members position on that point
4th column what the current position of the point was.
5th Column - blank as this would be for what the agreed solution is.
ie.

Col 1 - Royal Mail wish to close the canteen
Col.2 - Royal mails position is that the canteen is not making a profit and is under utilised.
Col 3. The CWU position was that as the Unit is pre P&L to close it was premature. The Union believe that the final decision should be delayed and a review carried out 4 to 6 weeks after Delivery Methods has been introduced.
Col 4 . Disagreement
Col 5. Blank

You and the manager get a copy as far as I am aware. The form itself lists where the disagreements go at the bottom of the page.
