royal mail cycles......design fault?

yesterday in our office a postie had an accident while out delivering,he came off his bike due to his handlebars snapping in half on his bike.he was hurt quite bad.i was juts wondering if there is a design fault with the cycles? what i mean by this is it turns out that the handlebars and the headset are one piece the handlebar doesn't run through the headset its welded onto the headset at both sides,on normal cycles the handlebars run through the headset.does anyone know if this has happened to any other bikes in other offices?

brothermagrew is your man for that, I have heard of handlebars breaking off but that was a right few years ago
