Assuming can prove sickness/hospital visits with medical bills etc., what is the policy of RM regards getting an extra day(s) in lieu of time lost from annual leave due to sickness?

If you have proof then they owe you the hours back as annual leave. It's literally written into employment law in this country.

They will only credit you a days holiday back in lieu of sickness with a medical note/fit note

Hmmm - just back from holiday yesterday. I was in hospital for a DVT - day patient - I didn't think to pick up a fit note - I have the medical reports and invoices. They'll have to do.

You can self cert if it's 7 days or less being sick.

Try doing that when you're on annual leave and it's probably more trouble than it's worth unless you have proof.
